Insurance

To keep medical care and billing costs down, payment for services and copayments required by your insurance provider are expected at the time of service. You will be responsible for payment of deductibles, co-payments and non-covered services. While the filing of insurance claims is a courtesy that we extend to our patients, all charges are your responsibility from the date services are rendered.

Please contact your insurance provider for information regarding our network status with them. Your providers rate of payment may depend on whether we are in network or out of network providers for them. Not all services are a covered benefit in all insurance policies. Your policy is a contract between you and your insurance provider. We cannot determine the benefits of your policy. Medicare and some insurance companies select certain services that they will not cover based on the contract with your employer. Payment for these services will be the responsibility of the patient.

We strongly encourage you to carefully read your insurance policy so you will know the conditions and circumstances of coverage available to you. Dr. Boyle is contracted with most major insurance plans, including but not limited to:
